Collection Instructions:

Allow specimen to completely clot (this can take up to 60 minutes). Centrifuge for 15 minutes. Transfer serum to a plastic transfer tube, and specify "serum" on the transfer tube label.

Rejection Criteria:

Specimens collected in SST (gel separator) tubes are not acceptable for testing.

Interference may occur with serum samples with patients diagnosed as having plasma cell dyscrasias and lymphoreticular malignancy associated with immunoglobulin synthesis, such as multiple myeloma, Waldenström's macroglobulinemia, and heavy chain disease. Results for these patients are typically suppressed.

Clinical Significance:

Vancomycin is a glycopeptide antibiotic. It is primarily used in the treatment of infections due to β-lactam or methronidazole-resistant gram positive cocci and bacilli. It may be used prophylactically in some patients who are at risk for endocarditis or when methicillin-resistant staphylococcus aureus or staphylococcus epidermidis is at risk. Since individual patients exhibit a high degree of variability in absorption and metabolism of vancomycin, therapeutic monitoring is recommended in certain patients. The major toxic reactions include “red-man syndrome”, nephrotoxicity, and ototoxicity when very high levels of the drug are present in the circulation.
